The exams are for students who need a physical prior to participating in sports and do not have a current physical on file or are unable to obtain one from their own primary care provider. If a child had a physical in the past 12 months, it is good for one year through the end of the month in which it was done.
Students must download the three required forms: Athletic Health History, Emergency Card and Health Appraisal from the school's Athletic Department web page. Students must bring the completed Athletic Health History and Emergency Card with parent/guardian signatures and the Health Appraisal form with parent/guardian signature of consent found on the bottom of the form.
Students who have had physicals from their primary care provider and have the required signed forms may forward them to the nurse’s office by mail. Due dates are as follows:
Varsity and Junior Varsity sports forms must be signed and dated on July 19 or after and are due on Aug. 2.
Modified sports forms must be signed and dated on Aug. 9 or after and are due on Aug. 23.
Varsity and Junior Varsity sports start Aug. 19.
Modified Sports start Sept. 9.
